Telemedicine Technologies in the Regional Mobile Application of the Healthcare Sector of the Tyumen Region "Telemed-72", as an element of Remote Monitoring of Citizens
In 2017, amendments were made to the Federal Law of 21.11.2011 No. 323-FZ "On the Fundamentals of Health Protection of Citizens of the Russian Federation," which established that the remote format of doctor-patient interaction is permissible in providing primary medical and sanitary assistance, specialized, and palliative medical care. Telemedicine consultations (online consultations) became especially relevant during the period of the novel coronavirus infection. During this time, citizens needed consultation regarding their health status, but visiting medical facilities posed a risk of contracting COVID-2019. To reduce the burden on doctors and receptionists, as well as to reduce the risk of contracting coronavirus infection when in contact with sick citizens, in the Tyumen region, first in a pilot mode, and soon on an industrial scale, the operation of the Telemed-72 mobile application, built on product platform "Medved.Telemed" reg. No. 21496 in the Russian software register. The Tyumen mobile application "Telemed-72" has elevated the accessibility and quality of medical services. One of the application's functional features is the ability to receive telemedicine consultations (online consultations). Thus, for citizens under medical supervision, as well as for all citizens during the pandemic, the opportunity for medical observation through online consultations is a highly relevant issue. The ability to talk with a doctor, consult, and get recommendations without leaving home allows patients to more closely monitor their health status and adhere to treatment tactics.

The competitive advantages of online consultations are exemplified by the remote support center based at City Clinic No. 8 in Tyumen, which has been operational since 2022. One of the remote center's functions includes telemedicine consultations using the "Telemed-72" mobile app. Last year, the Remote Support Center conducted about 13,500 telemedicine consultations using the "Telemed-72" app, accounting for nearly 75% of all such consultations in the Tyumen region. The center also evaluated the use of telemedicine technologies in outpatient care for glycemic control in patients with type 2 diabetes. Initial data: The study involved 40 patients with type 2 diabetes from November 1, 2022, to February 1, 2023, with an average age of 64 years. Patients were divided into two groups, comparable in age, gender, disease duration, and initial HbA1c: Group A: 20 patients received traditional in-person consultations, with additional remote endocrinologist support every two weeks to adjust their treatment. Group B: 20 patients, in addition to doctor-led face-to-face and remote consultations, had weekly tele-nursing with a nurse (survey, glycemia monitoring, decision-making for unplanned in-person or remote medical consultation). Both groups initially and after three months were examined for HbA1c and glycemia four times daily (fasting and before main meals) with self-filled patient diary entries two days before contact with medical personnel. Conclusions: Both telemedicine management approaches demonstrated a positive impact on glycemic control indicators in patients with type 2 diabetes. The implementation of tele-nursing by nurses has high potential to affect glycated hemoglobin levels, allowing timely, effective, and cost-efficient detection of signs of worsening conditions and organizing various types of medical assistance for patients

Finalist of the international award “We Are Together”, in the “Health of the Nation” category. The telemedicine service Medved.Telemed, on the basis of which Telemed72 was developed, became a bronze medalist among regional IT solutions for healthcare at the PROF-IT award.
