eSanjeevani - National Telemedicine Service (India)

Problem and implemented solution

Challenges: Healthcare delivery systems in countries like India face significant challenges that impede efficient and equitable access to medical services. These challenges stem from systemic issues that affect both the availability and quality of care across all levels of the healthcare system. They include: Non-availability/shortage of doctors and specialist doctors at the ground level. High burden on secondary and tertiary level health setups due to non-availability of services at the primary level, i.e., lack of gate-keeping. Lack of health record creation at the primary level. Lack of continuum of care. Implemented Solution: To tackle these challenges, the Ministry of Health and Family Welfare (MoHFW), Govt. of India selected Centre for Development of Advanced Computing’s (C-DAC) flagship and indigenous telemedicine platform titled eSanjeevani. C-DAC is a premier R&D (IT and Electronics) organisation under the aegis of the Ministry of Electronics and Information Technology, Govt. of India. It has been deployed in two variants: 1. eSanjeevaniAB-AAM: A Provider-to-Provider telemedicine system (assisted telemedicine) under Ayushman Bharat Scheme of Govt. of India is based on Hub and Spoke model and launched in November 2019. 2. eSanjeevaniOPD: A Patient-to-Provider telemedicine system to enable citizens to get outpatient services in the confines of their homes. This variant was launched during the peak of COVID 19, to provide safe Provider-to-Patient consultations. Since then, it has continued to grow in demand, even post-lockdown. The processes in eSanjeevani are modeled after traditional hospital-based outpatient departments (OPDs) to provide users with a familiar and effective experience. Designed with minimal steps and intuitive interfaces, the platform ensures ease of use for patients and healthcare providers. The digitalization of service delivery enhances accessibility and convenience, reducing physical barriers to care. This streamlined approach simplifies access to healthcare services and maintains the quality and efficiency of care delivery.

eSanjeevani, India's National Telemedicine Service, launched in 2019, has quickly expanded nationwide. Initially focusing on infrastructure and training, it was introduced as a monolithic platform for Health and Wellness Centers (HWC) and eSanjeevaniOPD. To meet evolving needs, eSanjeevani 2.0, with a cloud-based microservices architecture, was phased in between March and July 2023. Ongoing updates include advanced data analytics and enhanced patient management, ensuring the platform remains a vital and adaptive tool in India’s healthcare system.
